To help ensure your group has available to it the benefits it needs, CSA General Insurance Agency, Inc. offers multiple dental plans from various carriers. Not sure what plan best fits your needs? Here's a short overview of some of our group dental plans:

DHMO (Dental Health Maintenance Organization)

  • No deductibles
  • Virtually no claim forms
  • No annual or lifetime benefit maximums
  • No waiting periods

The plan also covers pre-existing conditions and orthodontics. It includes a co-payment schedule so your employees will know exactly how much each service will cost them at the time of service.

Voluntary Indemnity

You can offer your employees a Voluntary Indemnity Plan. With this plan, your employees pay the entire monthly premium. However, the employer may contribute any amount up to 49%. This plan provides one level of coinsurance for covered benefits and insureds may visit any dental provider of their choice. When visiting a contracted dental provider, insureds will incur less out-of-pocket costs due to negotiated contractual arrangements with the plan. The plan includes:

  • Access to any provider
  • Coverage for preventive, restorative, endodontics, periodontics, and oral surgery
  • Orthodontia available either as an in-network only scheduled/co-payment benefit or on a traditional coinsurance basis.

PPO

This plan also includes an in-network benefit level and an out-of-network benefit level. Over 81,000 dentist access points are available across the nation through DenteMax. The plan includes:

  • In-network and Out-of-network coverage levels
  • Diagnostic, Preventive, Restorative, Endodontic and Periodontal coverage
  • Orthodontia available either as an in-network only scheduled/co-payment benefit or on a traditional coinsurance basis.

The availability of some plans and the level of coverage provided for some covered services are dependent on group size and employer contribution level.
